Enhanced Access Patient Survey June to 15 July 2022
Primary Care Networks are groups of GP Practices working together as a network, and they are commissioned to deliver Enhanced Services. Enhanced services are provided over and above the core 8am-6.30pm contract.
Primary Care Networks have been asked to undertake patient engagement to ensure any new services meet the needs of the local population. It is proposed that any changes to services outside of the core 8am-6.30pm contract will be in place by October 2022.
Your GP Practice or Primary Care Network may already offer Enhanced Services and if this is the case then this survey will be an opportunity to check that the services offered best meet your needs.
We would be most grateful if you could take the time to complete a short survey by either clicking the QR code or by using this link: https://www.surveymonkey.co.uk/r/EnhancedAccessServicesSurvey
If you have any questions about this survey or would like to provide feedback in another way then please contact the Joined up Care Derbyshire engagement team ddccg.engagement@nhs.net

Please be polite we are trying our very best to help you.

Covid 19 Vaccinations
From the end of June 2022 we will be suspending our COVID vaccination service.
People in England who have been fully vaccinated against COVID-19 can demonstrate their vaccination status for international travel. A full course is currently two doses of any approved vaccine. Vaccine status will be available from:
the NHS App which you can download from app stores
It may take more than a week for your identity to be checked and verified so you can use these services.
If you cannot access these online services, and you have had two vaccines, you can request a paper letter from the NHS by calling 119. Only call 119 if you had your second dose more than five working days ago. It may take seven working days for the letter to arrive.
The Springs Health Centre is not able to provide you with a letter that shows your COVID-19 vaccination status.
When you're planning your travel, check the latest information on demonstrating your COVID-19 status when travelling abroad on the gov.uk website. Make sure there is time to get proof of your vaccination status before you leave.
